ASD ostium secundum: when simple becomes complex

Atrial septal defect (ASD) discovered in adulthood may be associated with concomitant left ventricular (LV) dysfunction given the frequent comorbidities in this patient population. Such patients should be carefully evaluated since closure of the defect may lead to immediate volume overload in a restrictive or dysfunctioning LV. We present the case of a young man admitted to our service due to decompensated heart failure in the context of ostium secundum ASD associated with severe left ventricular dysfunction.
